First Visit

Your initial appointment will consist of a consultation with Dr. Shah that allows you the opportunity to discuss your dental concerns and oral health goals. Afterwards, we will gather the necessary diagnostic information to help formulate a treatment plan. Dr. Shah will then spend time with you, one on one, explaining your diagnosis and treatment options. Occasionally, treatment can be performed the same day as the consultation. However, a complex medical history or treatment plan will require an evaluation and a second appointment to provide treatment on another day.

Please assist us by providing the following information at the time of your consultation:

• A completed Medical/Dental History Form
• Any recent dental X-rays (if applicable)
• A list of your current medical prescriptions and any other vitamins, supplements, etc.
• If you have dental benefits, please bring your card and any associated documents. This will save time and allow us to help you process any claims.

IMPORTANT: A parent or guardian must accompany all patients under 18 at the consultation visit.

Please alert the office if you have a medical condition that may be of concern (i.e. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) requiring antibiotics, or if you are on any medication (i.e. heart medications, aspirin, anticoagulant therapy, etc.)

X-Rays

If your dentist or physician has taken X-rays, you may request that they forward them to our office. We utilize cutting-edge Digital Radiography, so the X-rays can be emailed directly to our office and stored in your paperless Digital Patient Chart. If additional X-rays are necessary, they can be taken at our facility. (Our digital X-rays reduce exposure by 90% compared to traditional film X-rays.)

Service, Service, Service

Is there a patient somewhere who doesn't appreciate a little pampering? Our patients love to snuggle under soft blankets with a comfy head pillow and listen to their favorite music through Bose Noise-Canceling Headsets or watch their favorite show in HD on our large, Flat-Screen Monitors.
